Doctors and patients don't always see eye to eye when it comes to self-reported symptoms and what actually ends up in their medical record.
University of Michigan researchers analyzed symptoms in 162 patients.
They found more than 90-percent of those patients reported concerns about a glare but did not have it on their medical record.
The second most common inconsistency was eye redness with 80-percent having no medical record mention.
Only 30-percent of patient surveys matched their medical history.
